    Case Manager

    The Latino Community Development Agency (LCDA) is actively looking for hard working, creative, and dedicated individuals to join our agency and become integral parts of their team. Provides outpatient education and case management services to children, adolescents, and adult consumers with alcohol or drug abuse/dependence and mental health issues. This is a bilingual (Spanish/English) position that provides linkage for youth and families to their community resource needs.

    Position Requirements

    • Position requires education and experience sufficient for designation as a Certified Case Manager and Behavioral Rehabilitation Specialist per the standards adopted by the Oklahoma Health Care Authority and the Department of Mental Health & Substance Abuse Services 
    • Bachelor’s Degree in Psychology, Social Work, or related behavioral science field
    • Minimum of one (1) year experience in a mental health and substance abuse related field
    • Ability to effectively communicate verbally with Spanish/English speaking consumers
    • Proficiency using Microsoft Word

    Essential Duties & Responsibilities

    • Provides services that are recovery focused, strengths based, culturally competent, consumer centered, and co-occurring capable.
    • Interacts with consumers in a respectful, genuine and professional manner.
    • Conduct Initial Intake (screening) to identify potential clients.
    • Conducts assessments, which includes gathering information about the consumer’s symptoms, behavior, functioning, history, needs, goals, and preferences as part of case management or rehabilitation services.
    • Works as part of treatment team to meet the needs of the consumer in a timely and thorough manner.
    • Maintains records in a timely manner as required by agency, state or federal funding sources, and accreditation authorities. Completes all required forms and is consistent with requirements. Document all activities directly or indirectly related to serving youth, adults and families in the program on a daily basis.
    • Provides case management or rehabilitation treatment services using agency-approved, evidence-based practices that are consistent with the current treatment plan and with consumer needs and preferences, including Individual Rehab, and group Rehab. Topic areas might also include, but are not limited to: domestic violence, communication skills, disease concept, 12-step recovery model and relapse prevention.
    • Ensures confidentiality of all personally identifiable information. Obtains necessary releases and consents and advises the consumers about their rights and responsibilities.
    • On a bi-monthly basis, participates in Clinical group supervision with supervisor.
    • Report any issues of concern to Supervisor and/or Director of Programs.
    • Conduct home, school or other community visits to increase client and family engagement.
    • Spends at least 50% of work hours providing direct services to youth, adults and families to maintain position status. Ensures effective and consistent delivery of services. Demonstrates flexibility in scheduling to accommodate client needs for evening appointments. Return phone calls, text messages promptly. Checks and responds to emails daily.
    • Conducts outreach to identify potential clients. Submits information on new clients and updates to data entry on a monthly basis.
    • Make referrals to therapists for individual and group intervention. Consults with other program staff, as needed, to facilitate referrals.
    • Familiar with the ASI, T-ASI, ASAM, and other instruments commonly used by substance abuse professionals and is certified/competent in their administration and interpretation.
    • Position may possess a certification for Alcohol and Drug Counselor (CADC) with approval from program manager of substance abuse department.
    • Must complete ODMHSAS Behavioral Health Case Management Certification
      process as scheduled by program manager or supervisor and maintain certification.
    • Performs other duties consistent with training, experience, or assigned by supervisor.

    • Monday through Friday. Varied, May Include Evenings and Weekends
